Student Information Form

We are excited to have you join us for the Student Ministry service at New Hope Church. Doors open at 5:45 pm, service starts at 6:00 pm and doors close at 7:45 pm.  Simply fill out the info below and we get you set up. When you arrive, head over to check in to get a name tag.  If you have any further questions, please email us at students@newhope.org. Please enter the adult's information on the top section of this form. You may enter additional adults and children as necessary by clicking the add buttons under Household members.
